Below you will find documents, templates, instructions, and links to help you understand and participate in institutional assessment. Forms

Departmental Assessment Plan Summary Overview: An Excel spreadsheet used by departments as a checklist to determine if each aspect of the Assessment Summary Plan has been adequately addressed. 

Budget Form: Used by departments to plan annual budget requests and tie those requests to the instution's mission

For Service and Support Areas

Assessment Plan Summary (APS) Part I: A fillable form for service and support areas to record goals and assessment tools for each year.

Assessment Plan Summary (APS) Part II: A fillable form for service and support areas to record the results and modifications related to the goals stated in Part I for that year.

For Educational Programs

Syllabus Template
The Syllabus Template provides a common format for course syllabi that ensures that key elements for assessment and student & academic services information are included in every course. For assessment purposes, it provides instructions for linking course student learning outcomes to program learning outcomes; for student services, it provides information such as disability services and the writing center. See MAT-115 syllabus example by Dale Gaddis.

Course-level Reporting

Course-level SLO Spreadsheets: This Excel file provides a two spreadsheets: one for computing SLO results using a rubric (4-trait) and a second worksheet for computuing SLOs using pass/fail criteria. This is a tool for instructors to compute SLO results for a particular course. It is not intended for institutional reporting.

PSLO Collector (link): Click the "PSLO Collector" link to the left to complete a form that collects course-level data for departmental use. This information cannot be retrieved by instructor name or section, but departments will receive reporting by course.
